Disodium hydrogen citrate syrup uses

Also known as sodium acid citrate, disodium hydrogen works as an alkalinizer. It is sold under various brand names globally. One of the primary uses of disodium hydrogen is to reduce the acidic level of urine. This is achieved by raising the pH level of urine. The exact composition of disodium hydrogen in a specific branded product may vary.

Each 5 ml of the solution usually contains around 1.25 to 1.53 grams of disodium hydrogen citrate BP. The blended solution is usually manufactured as a syrup to be taken orally. Many disodium hydrogen branded syrups contain sweeteners and flavors for easy consumption.

Urinary tract infections (UTIs) – Disodium hydrogen can be prescribed during the treatment of urinary tract infections. It can also work for related inflammation of cystitis, urethritis and pyelitis. It is effective in reducing the burning sensation, pain and discomfort experienced by UTI patients during urination. Since disodium hydrogen increases the pH level of urine, it can enhance the effectiveness of specific antibiotics such as sulfonamides and fluoroquinolones. It is important to note that disodium hydrogen does not directly treat the infection.

Kidney stones (urolithiasis) – Disodium hydrogen prevents the formation of uric acid and cystine stones. With its alkaline nature, disodium hydrogen can also dissolve small existing uric acid stones. However, it is not effective on calcium-based stones.

Gout and high uric acid levels – Disodium hydrogen can help remove excess uric acid through urination. Hence, it reduces the risk of formation of uric acid crystals in the joints.

Metabolic acidosis / renal tubular acidosis – With its alkaline effects, disodium hydrogen can also help with acute or chronic metabolic acidosis. This can be related to infections, issues with the kidney and dehydration.

How disodium hydrogen works?

As mentioned earlier, disodium hydrogen works by increasing urinary pH levels. After it is absorbed in the body, it is metabolized to bicarbonate. This increases the removal of free bicarbonate ions in the urine, which subsequently increases urinary pH. The alkaline increase is usually around 6.5 to 7.5 or higher. Disodium hydrogen has a mild diuretic effect. It increases the solubility of uric acid crystals.

Disodium hydrogen dosage

The dosage is set by the doctor, based on the specific symptoms and condition of the patient. Common reference range is 15-30 ml two to four times a day for adults. For children in the 6-12 age group, the reference dose of disodium hydrogen is 10-15 ml three to four times a day. Younger children’s dose is prescribed by a pediatrician. Reference range is around 2-5 ml.

Side effects of disodium hydrogen

This is usually well tolerated. However, there can be side effects like nausea, vomiting, diarrhea, flatulence and unpleasant taste. Tiredness and mood changes can also occur in specific cases. Some less common yet serious side effects have also been noted such as metabolic alkalosis, hypokalemia (low potassium), hypocalcemia, sodium overload, allergic reactions and gastrointestinal bleeding or ulceration.

Precautions, contraindications, and drug interactions

Disodium hydrogen is usually not recommended for patients with severe kidney impairment. It should also not be used for patients with edema, high blood pressure, untreated Addison’s disease, severe myocardial damage, acute dehydration, heat cramps, existing metabolic alkalosis and any known allergy. Caution is advised for people with diabetes, pregnancy, breastfeeding, heart disease, liver issues and low calcium.

Disodium hydrogen may react with specific medications like antibiotics, quinidine, amphetamines, ephedrine, salicylates, barbiturates, corticosteroids, potassium-depleting diuretics, methotrexate and tetracyclines. It is important to consult your doctor before taking disodium hydrogen syrup. Also, note that disodium hydrogen is not a substitute for antibiotics in UTI cases. It is also not a definitive treatment for large kidney stones.
