A Rare and Challenging Pediatric Case Management of a Congenital Laryngeal Cyst by Apollo Multispeciality Hospitals, Kolkata

A complex and rare pediatric surgical case was recently managed successfully at Apollo Multispeciality Hospitals, Kolkata. The case involved a two-year-old girl referred with life-threatening airway obstruction caused by a large congenital laryngeal cyst. This condition is exceptionally rare, with a reported incidence of only 1.8 per 100,000 live births. The patient, a two-year-old girl, was admitted with a history of severe respiratory distress, poor feeding, and failure to thrive. She also presented with occasional episodes of bleeding from her mouth. Clinical evaluation revealed a diagnosis of a congenital laryngeal cyst, which had grown to a size that completely obstructed her voice box and windpipe. This critical condition required immediate and definitive surgical intervention to save the child’s life.

The surgical procedure presented a unique set of critical challenges, primarily because the large cyst made a standard intubation—the process of placing a breathing tube into the windpipe—impossible.
 
To secure the child’s breathing immediately, the surgical team first performed an urgent tracheostomy. This procedure involved creating a temporary opening in her windpipe to bypass the obstruction and allow her to breathe safely. Once the airway was stabilized, the primary procedure of removing the cyst was carried out successfully using an advanced endoscopic coblation technique.
 
Following the surgery, the child made an uneventful and complete recovery. She has since resumed normal feeding and speech and is now entirely free from respiratory distress. Her condition is stable, and she is expected to be discharged from the hospital soon. The successful outcome of this particularly rare case serves as a testament to the seamless coordination and clinical excellence of the entire medical team involved. It underscores the critical role of specialized expertise and advanced surgical techniques in providing life-saving care for children with complex and rare conditions.
